Liability Policy:  By booking a session with Cosmic Healing With CJ, YOU, the client, are agreeing to the following terms and conditions of purchasing services from Cosmic Healing With CJ. I understand that Reiki and Energy Healing are simple, gentle, hands-on energy techniques that are used for stress reduction and relaxation.  I understand that Intuitive Coaching sessions, Meditation and Intuitive Readings are for entertainment purposes only, and does not constitute a psychic reading, nor does the session imply or guarantee that anything discussed during the session is a prediction of future events that may or may not occur during the client's lifetime. I understand that Cosmic Healing With CJ, its owner(s), employees, independent contractors, affiliates and/or authorized representatives are not medical professionals and cannot diagnose conditions nor prescribe or perform medical treatment, prescribe substances, nor interfere with the treatment of a licensed medical professional. I understand that the products or services provided to me by Cosmic Healing With CJ do not take the place of medical care. I understand that it is recommended that I see a licensed physician or licensed healthcare professional for any physical or psychological ailment I may have. I understand that the products and services provided by Cosmic Healing With CJ can complement, but cannot replace any medical or psychological care I may need and/or be receiving. I acknowledge that it is not possible to estimate the number of sessions I will need to resolve the issue(s) which necessitate my decision to utilize the services of Cosmic Healing With CJ. I further acknowledge that it is not possible to guarantee any specific outcomes for any sessions I receive from Cosmic Healing With CJ.  I voluntarily assume the risk of injury, accident or death, which may arise from the use of Cosmic Healing With CJ. I, and any of my heirs, executors, representatives or assigns hereby release from all claims or liabilities for personal injury or property damages of any kind sustained as a result of receiving services from Cosmic Healing With CJ, and from any advice provided by an owner, employee, independent contractor, affiliates and/or any authorized representative. I agree that this intake form and Waiver is in effect for all Cosmic Healing With CJ sessions I receive and will not expire unless requested by either party.  

Acknowledgement:  By scheduling a session with Cosmic Healing With CJ, you acknowledge that Cosmic Healing With CJ, and its owner(s), employees, contractors or authorized representatives are not medical professionals and the products and services offered by Cosmic Healing With CJ, including but not limited to, Reiki, Crystal Healing, Sound Healing, Meditation, Intuitive Coaching, and Intuitive Readings, do not replace medical care from a medical professional. The products and services offered by Cosmic Healing With CJ does not replace psychiatric care. The products and services offered by Cosmic Healing With CJ cannot diagnose, treat, cure, or prevent an illness. Cosmic Healing With CJ is not liable for any decisions made by you the client following a session concerning your health and/or your life in general. Everything discussed during the session is to be kept completely confidential by both parties. If you are taught anything during the session, the information is not to be repeated or replicated as your own.  

Privacy Notice:  No information about any client will be discussed or shared with any third party without written consent of the client or parent/guardian if the client is under 18.  

Policies for First Time Clients:  First time clients must complete the Client Intake Form. Completion of the intake form is required before the first session can be conducted. The client's scheduled session time will not be extended due to them needing to complete the form upon arrival for the appointment.  

Payment Terms:  Cosmic Healing With CJ accepts Credit and Debit Cards, PayPal, Venmo, Cashapp, and Cash.. We do not accept personal checks. - Payment in full is due at the time of service.  

Late arrivals:  Appointments will not extend past the originally scheduled appointment end time. Clients will not receive a refund for any portion of their missed appointment time.  

No Shows:  A client is considered a no-show if 20 minutes has elapsed since the start of their appointment and they have not arrived at the appointment location. No shows will not be refunded and will forfeit any monies paid to Cosmic Healing With CJ for the session. No-shows will be charged for the balance due immediately via the payment method on file used to secure the appointment.  

Cancellations:  Cancelled appointments will not be refunded. Clients are encouraged to reschedule rather than cancel appointments. Clients have up to 48 hours prior to their session start time to reschedule their appointment. Please note: After TWO (2) consecutive reschedules, the client's appointment will be marked as cancelled, and the client forfeits any monies that have been paid to Cosmic Healing With CJ. If the client wishes to schedule another appointment, he or she will not be credited for any monies paid to secure the rescheduled appointment(s).  

On-line Privacy:  At Cosmic Healing With CJ, we are committed to protecting your privacy as an online visitor to our website. We use the information we collect about you to maximize the services that we provide to you. We respect the privacy and confidentiality of the information provided by you and adhere to the privacy laws. Please read our privacy policy below carefully. You may change your details at any time by advising us in writing via email. All information we receive from our customers is protected by our secure server software encrypts all customer information before it is sent to us. Furthermore, all customer data collected is secured against unauthorized use or access. Credit card information is not stored by us on our servers.  

Storage and Security of Your Information:  We receive and store information you enter on our website or give us in any other way from time to time. You may provide basic contact information such as your name, phone number, address, and email address to enable us to send information or process your product order and we may also collect additional information at other times, including but not limited to, when you provide feedback, change your content or email preferences, respond to a survey, or communicate with our customer support.  We may use personal information collected from you for the purpose of providing you with direct marketing material, updates regarding our website and information in the form of a newsletter. This will only apply if you have registered or subscribed to such publications by registering your details with us. However if you wish to cease receiving any such information you may let us know either by email or unsubscribing at any time and your request will be actioned immediately.  Individual profile and company details are not used for any other purpose. Details are only supplied to a third party supplier when it is required by law, for goods or services which you have purchased or to protect our copyright, trademarks and other legal rights.  We respect the privacy of our online visitors. We may collect information on or through this website that can personally identify you. For example, we collect personally identifiable information which you volunteer to us to respond to visitor questions and comments about us and our products and services, and to mail e-newsletters ("Personal Data").  We will use all reasonable means to protect the confidentiality of your Personal Data while in our possession or control. We will not knowingly share any of your Personal Data with any third party other than our service providers who assist us in providing the information and/or services we are providing to you. To the extent that we do share your personal information with a service provider, we would only do so if that party has agreed to comply with our privacy standards as described in this privacy policy.  Any non-personal information, communications and material you send to this website or to us by email, or which we obtain from third parties without promises of confidentiality, may be kept, used and disclosed by us on a non-confidential basis.
